What we collect
When you use the form we collect your full name, mobile phone, email, ZIP code, and — if you choose — the date of your accident and a short note about where it hurts. We also record the page you submitted from.
How it is used
Submissions are sent to Atlas Injury Network's case-management system and to a monitored inbox so a case manager can contact you and help connect you with care in your area.
Sharing
Your information is used to coordinate your care request. It is not sold.
